Palatine Celtic Cup Tournament Forms
Illinois Medical Release Form
All non-IYSA teams must use this form. We do not need copies of insurance cards and the forms do not need to be notarized. Teams that are registered through the IYSA, do not need to provide us with copies of the medical release forms, but you must have copies available at all games.
Guest Player Form
--Once you click the above link you will be taken to the Tournament Center and you must create a username & password --Complete Guest Player/Player Loan Form --After completing all forms, print confirmation. This is what you send to us or bring to final registration.
